NEW PATIENT REGISTRATION
Welcome to our practice!
To register as a new patient of the practice you will be required to complete both of the below forms
- New Patient Health Questionnaire and
- HS200 form
We collect your personal details, including your name, date of birth and contact information to establish your patient record. This process helps us to ensure that your medical history is accurately and securely transferred to our practice.
In addition to the above forms you will also be required to take some supporting documentation listed below
- Proof of address – e.g. a utility bill dated within the last 3 months or Immigration status documentation
- Photographic ID – Please provide a Passport or Driving license. (For children with no passport you must provide a birth certificate)

REGISTERING A BIRTH
To register a birth in Northern Ireland, you must fill in the form linked below and take it to the local Registration Office.

(Note that a birth registration cannot be easily changed, so make sure to fill in the registration form carefully!)
Once you have given this form into your local registration office, the registrar will give you an Infant Registration Card (HS123) which you can take to the practice to register your baby with us. It is vitally important you do this promptly so your child can have a unique Health and Care number, be issued prescriptions if required, be enrolled for the immunisation programme and avail of Infant check ups with the GP and Health Visiting team.

REGISTERING A DEATH
In the unfortunate event that a person has passed away, there are three things that must be done in the first few days;
- Get a medical certificate from your GP or hospital doctor (this is necessary to register the death)
- Register the death within 5 days. You will then receive the necessary documents for the funeral
- Make the necessary funeral arrangements
If the death has been reported to the coroners office, this is usually because the deceased had not been seen by the doctor within 28 days before death, the death was not caused by natural illness or the cause of death was unclear, sudden or supicious.
If a death has been referred to the coroner by a GP, you should not proceed to make funeral arrangements until the coroner has given consent to proceed. The coroner can give consent for burial or cremation to take place before the death is registered.
The death can only be registered and a death certificate obtained after the registrar has received the nescessary certificate from the coroner.
When the registrar receives the certificate they will contact a relative of the deceased and ask them to call in at the office to register the death.
You can register the death if you are a relative, a witness to the death, a hospital administrator or the person making the arrangements with the funeral directors.

TEMPORARY REGISTRATION
If you are ill while away from home or if you are not registered with a doctor, but need to see one, you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.
You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice.
To register as a temporary patient simply contact the practice. We do not have to accept you as a temporary patient although we do have an obligation to offer emergency treatment. You cannot register as a temporary patient if you are already registered at a practice in the town or area where you are already registered.
ACCOUNTABLE GP
From 1st April 2015, GP practices are required to allocate all patients (including children) with a named accountable GP.
The named GP is responsible for the co-ordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.
- Patients do not need to see their named GP when they book an appointment with the practice.
- Patients are entitled to choose to see any GP or nurse in the practice.
