REQUESTING MEDICAL NOTES


If you require a copy of your medical notes please contact our medical reception team either via telephone 02880761242 or via a written request out lining what you specifically need e.g. encounter report, last three years medical notes ect.


Please allow up to 28 days for processing your request.


If you have requested the same medical notes previously, no matter what the circumstances- ie via your own request/ a solicitor request or third party request, this will incur a fee which the medical reception team will be able to advise you off. 


 

SUPPORTING INFORMATION REQUESTS


If you require a copy of your medical records for the purpose of completing a benefits application (e.g. PIP, Universal Credits), please make contact with our medical reception team on 02880761242 and we will endevour to have these ready for you within 14 days. If this information has been requested before please bear in mind there may be an administration fee charged. 


GP's do not provide support letters for the purpose of benefit applications. Please do not be offended if your request is declined. The Department For Work and Pensions have a statutory obligation to gather medical evidence if it is needed for a claim. In such cases they will write directly to the GP practice with a specific form requesting the relevant information, which we complete and send back to them. 


 

PRIVATE MEDICAL FORMS AND EXAMINATIONS


Some requests for medical forms, reports or examinations are not covered under the NHS General Medical Services (GMS) contract. This means that GP's are not obliged to complete them and they are classes as private (non-NHS) work. 

Examples may include

  • Insurance reports or claim forms
  • Employement or occupational health forms
  • Driving medicals or fitness to participate certificates
  • Holiday cancellation certificates
  • Private medical examinations

As this work falls outside of NHS services, a private fee will be charged if the GP agrees to complete the request. Our reception team can advise you of the cost in advance and provide details of payment methods. Fee's are based on guidance from the British Medical Association (BMA).


Please note that in some cases the GP may decline the request. If this happens, you may be able to have your needs met through a private GP or occupational health service. 


We thank you for your understanding that NHS time must be prioritised for essential medical care. 


Please allow a 28 day turn around for any private forms/examinations that the GP agrees to complete.
