New Patient Registration
The practice list is open but because of the limitations of the practice premises we can only register patients who move into the practice area of the practice premises and subject to approval of the application.
Application is made by contacting the practice and providing the information requested by the receptionist- this will then be brought to the weekly practice meeting where the GPs can decide if the registration criteria has been met. You will be informed in writing of the decision, if you are accepted onto the list you will be asked to provide your medical card or application form, signed by the applicant or by a person authorised by the applicant to sign on his/her behalf- they are also particular items you will be advised to produce to ensure your application is approved by BSO.
When accepted you will be registered with the practice and not an individual GP. You are however, free to request an appointment with any of the doctors.
Clinical services are provided to people who are staying temporarily in the area or require emergency or immediately necessary treatment, unless the patient refuses consent.
Change of Personal Details
Please notify the surgery as soon as possible of any change of name, address or telephone number, so that your records may be kept up to date. If you leave the area, you will need to register with another doctor, by completing the relevant part of your medical card and enquiring at their surgery of their registration procedure.
Guide to GP Services
The Royal College of General Practitioners has produced a useful guide for patients about the services on offer at GP Surgeries and how to access them. You can download the guide below.
