New Patient Registration

OUR PRACTICE LIST IS CURRENTLY CLOSED UNTIL FEBRUARY 2024

Carrickmore If you have moved into the immediate area and only if you do not have a GP in this area- please contact the Health Centre and it can be discussed for consideration of registration.

Application is made by contacting the practice and providing the information requested by the receptionist- this will then be brought to the weekly practice meeting where the GPs can decide if the registration criteria has been met. You will be informed in writing of the decision, if you are accepted onto the list you will be asked to complete an application form, signed by the applicant or by a person authorised by the applicant to sign on his/her behalf- they are also particular items you will be advised to produce to ensure your application is approved by BSO.

When accepted you will be registered with the practice and not an individual GP. You are however, free to request a Triage appointment with any of the doctors if available.

Clinical services are provided to people who are staying temporarily in the area or require emergency or immediately necessary treatment, unless the patient refuses consent.

Change of Personal Details

Please notify the surgery as soon as possible of any change of name, address or telephone number, so that your records may be kept up to date. 

If you leave our Practice Area ( which has a boundary of 10 miles),You MUST register with a doctor in your new address area.  You can enquire at the New surgery of their registration process.
